Dexter and Royce are two addicts who drift through life in a dope-filled haze until Royce’s girlfriend, Matilda, ODs on their stash. In a panic, the two bury her in a basement where they stumble upon a Satanic cult performing a ritual sacrifice. Matilda suddenly wakes from her coma and in the distraction she and the boys manage to escape. The Satanists however, think they brought Matilda back from the dead, and they want her back.

Meanwhile, the local drug kingpin is after them for the money they owe on the stash they were supposed to be selling for him. The guys figure the only way the can pay back Nigel is to rip off the town’s eccentric millionaire. Too bad Royce left the plans for the job and the safe code back at his apartment. Before the night is through, a band of armored midgets will have come to their rescue. And that’s just how weird it gets in Weirdsville.

The film follows Mr. Smith (Clive Owen), a drifter who appears to have an extensive military background and a fondness for carrots who wants nothing more than to be left alone. Smith finds himself embroiled in a complex political conspiracy once he aids a pregnant woman who is going into labor while being chased by a hitman. After the woman is killed, he takes the baby and goes on the run with a lactating prostitute named Donna (Monica Bellucci). The unlikely family is trailed by the intelligent and ruthless Hertz (Paul Giamatti) and his army of thugs. A plethora of elaborate gunfights ensue, between which Smith pieces together the real story: a United States Senator who strongly supports gun control had been breeding babies for their bone marrow to treat his cancer, and a prominent gun baron has contracted Hertz to kill the babies. After Smith kills the senator he is captured by Hertz who tries to get him to tell where the woman and child are. He soon manages to escape and kill Hertz and returns to the baby and Donna. The film ends with him taking out a band of robbers who attempt to rob the restaurant where they are reunited.

Jigsaw (Tobin Bell) might be dead, but his traps live on in this fifth Saw entry, which finds the series' production designer David Hackl at the helm for his debut directing gig. Costas Mandylor reprises his role as Hoffman, the detective whose involvement in the string of grisly murders turns out to be more than meets the eye. Picking up where the fourth entry left off, Hoffman is revealed to be a hidden accomplice in Jigsaw's grisly games of death, which look to be continued by the lawman. The plot bounces back and forth from the beginning of their relationship to the present, as Agent Strahm (Scott Patterson) delves farther into the case. Also in the cast are Dexter's Julie Benz and One Missed Call's Meagan Good as victims of another one of Jigsaw's lessons in morality. Saw IV scribes Patrick Melton and Marcus Dunstan return to pen the screenplay for the Twisted Pictures/Lionsgate co-production.
